What is RF microneedling?
RF microneedling is microneedling with an energy source attached. The tip holds an array of small electrodes. Those electrodes puncture the skin, then release radiofrequency energy at a set depth.
The FDA device description is specific. Each microneedle produces a small area of intense heat at its tip. That heat changes the tissue around it. The American Academy of Dermatology puts it in one line. The heat travels down the needles.
How does RF microneedling differ from standard microneedling?

Radiofrequency arrived within the past decade. A 2023 review in Aesthetic Surgery Journal Open Forum by Shauly and colleagues names the purpose. Thermal injury and dermal heating. Thermal injury drives fibrotic remodeling, and remodeling produces the tightening effect.
So the two procedures share a delivery method and split on physics. One opens a channel. The other heats the tissue beside it.
Why insulated needles change the outcome
Insulation decides where the heat lands. On an insulated tip, only the lower section of the needle releases energy. The shaft passing through your epidermis stays inert.
The 2023 review states that insulated RF microneedling delivers focused injury to the dermis and spares the epidermis. That distinction matters most for pigment.
Not every device is insulated. The same review lists the Vivace as a non-insulated cartridge carrying 36 needles at 0.3 mm diameter. Infini and Morpheus8 are listed with insulated tips. Ask your provider which system they use. The FDA now recommends that exact question.

How deep does each one actually go?
Depth is the number that separates these categories. It is anatomy, not marketing.
The 2023 review gives a working map for RF microneedling by region.
- Forehead and temples: at least 1.5 mm
- Cheekbone: 1.0 mm
- Nasal side walls: 2.0 mm, named as the maximum depth
- Around the mouth: 0.5 mm
- Neck: 1.5 mm
One listed device, the Genius, reaches 0.5 to 3.4 mm.
Standard microneedling usually runs shallower. The same review notes aestheticians often stay at or under 0.5 mm. Most states do not permit depths past the epidermis without further certification.

What the FDA said in October 2025
On 15 October 2025 the FDA published a safety communication on RF microneedling. It is the most important document in this comparison, and most articles still skip it.
The agency reported serious complications: burns, scarring, fat loss, disfigurement, and nerve damage. Some cases required surgical repair. Its evaluation is ongoing.
One line does the heavy lifting. The FDA calls RF microneedling "a medical procedure, not a cosmetic treatment". It states these devices should not be used at home. It also asks patients to find a trained licensed provider, and to ask which device will be used.
The American Academy of Dermatology responded on 16 October 2025. ASLMS and ASDS issued a joint response. Their collective experience finds RF microneedling safe and effective in proper settings. They asked members to name every listed complication in consent forms.
Read both together. The technology works in trained hands, and the margin for error is narrower than the category's reputation suggests.
Does RF microneedling cause hyperpigmentation in darker skin?
It can, and the mechanism is documented. The 2023 review calls dyspigmentation a rare side effect that mainly concerns darker pigmented patients. It occurs when thermal energy sits too close to the epidermal layer. Ultraviolet exposure after the procedure is the other trigger. The listed correction is lower energy and shorter pulse duration.
A 2021 systematic review in the Journal of Clinical and Aesthetic Dermatology reviewed 51 articles covering 1,029 patients. Gowda and colleagues found microneedling relatively safe overall. They also called post inflammatory hyperpigmentation a realistic side effect, particularly in patients with darker skin. It can appear after multiple sessions.
The AAD states RF microneedling is safe for all skin tones. The same page adds a caveat worth reading twice. The supporting studies in darker skin tones were small, and more research is needed.

What downtime and pain look like
Standard microneedling recovers quickly. The AAD calls downtime minimal. Expect redness on lighter skin or darker shades on deeper skin. Skin can feel sunburned for a few days up to a week. Makeup waits 24 hours.
RF microneedling asks for more. The 2021 systematic review reports an average pain score of 5.4 out of 10 during the procedure. That figure comes from one study of 15 subjects treated with RF microneedles. Topical anesthetic is standard. The same review notes faint bruising in three of 60 patients after three monthly sessions.

Which one fits your problem?
- Laxity along the jaw and neck. Heat is the reason RF exists. Fibrotic remodeling produces the tightening, and no unheated device reproduces it. See sagging skin causes.

Safety first: who should skip microneedling
The AAD lists conditions that rule it out. Skip microneedling if you:
- Heal slowly or poorly
- Have a history of keloid scars
- Have a weakened immune system
- Are being treated for skin cancer in that area
- Are receiving radiation
Postpone it for a tan or recent sun, a skin infection, or painful cystic acne.
