Microneedling Pen vs Dermaroller: The Mechanical Difference

A pen stamps vertically. A roller enters and exits on an arc. That one difference produces almost every other, including the one about sterility.


Microneedling pen vs dermaroller is a question about motion, not about needle count. A pen stamps needles straight in and straight out. A roller drags a studded drum across skin, so every needle enters on one arc and leaves on another.

That single mechanical difference produces almost every other difference between the two: what the opening looks like, whether depth is adjustable, and whether the part that touches your face can be sterile. This page works through the mechanics first, then the tradeoffs that run the other way.

Two mechanisms, described properly

The roller: entry and exit on an arc

A derma roller is a handle and a drum. The drum carries fixed needles and you push it across the surface of the skin.

The needle at the bottom of the drum is the only one at full depth, and it arrives there while the drum is still turning. So the needle enters at an angle, sweeps through a short arc inside the skin, then exits at the opposite angle. The opening it leaves is longer than the needle is wide.

Practitioners commonly describe roller entry as angled rather than vertical, and describe the resulting opening as more of a tear than a puncture. Nothing about that is a manufacturing flaw. It is what a wheel does.

The pen: vertical, repeated

A microneedling pen holds a needle cartridge on a motor. The motor drives the array down and pulls it back on a straight line, many times a second, while you move the pen across an area.

Entry and exit follow the same path, so the opening is closer to the width of the needle itself. Depth is set by a collar rather than fixed by the tool you bought, and the rate is set by a separate control.

That is the whole mechanical case for a pen. It is a real difference and it is smaller than the marketing around it.

Why the angle matters more than the needle count

Roller marketing counts needles. A drum with 540 needles sounds like more device than a cartridge with fewer.

Count is the wrong number. What matters is the shape of each opening and how the skin around it was treated on the way in. A needle that travels sideways while it is inside skin loads the tissue laterally, and lateral load is the mechanism behind most home microneedling marks.

The same physics explains the single most common home technique error with a pen: dragging it. A pen held still and tapped is doing pen mechanics. A pen skated across the face is doing roller mechanics with a motor attached, which is worse than either.

Needle width is the spec both categories bury

Depth is how far the needle travels. Diameter is how wide it is. Brands in both categories quote one and stay quiet about the other.

For scale, a scalp hair is usually 50 to 100 µm across. Those are general literature values used for comparison, not measurements of your skin.

Two devices set to the same 1.0 mm are not doing the same thing if one needle is twenty five times wider than the other. Ask for both numbers from anyone selling you either format. The depth guide takes that argument apart setting by setting.

Sterility is the difference that actually decides it

Here is the part that outranks mechanics in practice. A roller drum is normally kept, rinsed and used again. A pen cartridge is normally binned.

Sterility is not a property a needle holds. It is a state a needle can only leave, so there is no return leg. Alcohol at a bathroom sink does not bring it back, and neither does hot water or a spray in a cup.

The distinction matters more than the words suggest. Clean and sterile are different standards, and a home bathroom can reach the first one on a good day. Only the second one is the standard for an object you are about to push through a barrier.

Reuse also stacks a second problem on the first. A drum that has been through skin, dried on a shelf and rolled again is carrying whatever it collected, straight back through the barrier you just opened. That is the risk practitioners raise most often about home rolling, and it is a hygiene argument rather than a mechanical one.

What a pen does not fix

A pen is not automatically the safer object. Several things get better and several get worse.

  • A motor removes the effort that used to limit you. You can cover a face faster, which also means you can overwork an area faster.
  • Adjustable depth is a control you can set wrong. A fixed roller cannot be turned up on a bad day.
  • A pen at 1.5 mm in a heavy hand is not gentler than a roller at 0.25 mm. Mechanism does not outrank setting.
  • Consumables are a real running cost. A roller has none, and pretending otherwise is dishonest.
  • Neither is a clinic appointment. Both are cosmetic devices, and anything you would call treatment belongs with a dermatologist.

The things a roller does better

A comparison that only runs one way is an advert. Three points go to the roller, and they are real.

  • Cost of entry. A roller is cheap enough to try, and a pen plus cartridges is a larger decision made before you know whether you will keep it up.
  • No power, no charging, no cartridge to run out of. A roller in a drawer works in three years. A device with a battery is a device with a battery.
  • Large flat areas. A drum covers a wide stretch quickly, and covering ground is the one job a wheel was designed for.

None of those cancel the reuse problem, and none of them are arguments about what happens inside skin. They are arguments about whether the tool fits your life, which is a fair thing to weigh.

Where the MTS sits, stated plainly

The MTS is a pen. The cartridge is driven vertically, the collar travels 0.5 to 1.5 mm with no clicks or stops, and intensity is a separate control with six levels.

One thing is unusual enough to name. The ampoule is loaded into the cartridge and released at the moment the needle meets skin, so the formula and the opening arrive together rather than in two steps. The coupling is drawn in section on the MTS device record, which also names the parts of the internal geometry the manufacturer has not published.

What re:tones does not say about any of this is equally specific. No clinical study, no consumer panel, no efficacy percentage, no absorption multiple and no time to result. The full ledger is on the science and claims record.

Choosing between them

  1. If you will reuse the needle, choose neither. This is the rule that decides most cases. A tool you will not replace is a tool that will eventually be dirty.
  2. If you need different settings for different areas, a pen is the only option. Eyelid skin and forehead skin are not one calibration.
  3. If cost is the constraint, be honest about the running cost too. A cheap roller you replace properly costs more than the sticker suggests.
  4. If your goal is a scar, a burn or a diagnosed condition, choose a dermatologist. Neither format is a substitute for a procedure.

Is a microneedling pen better than a derma roller?

A pen enters vertically and lets you change depth per area, and a roller does neither. Better depends on what you do with it, since a pen set deep in a heavy hand is not gentler than a shallow roller. The reuse question usually settles it before the mechanics do.

Do derma rollers cause more damage than pens?

Angled entry and exit load the skin sideways, and lateral load is the mechanism behind most home needling marks. That is a description of the motion rather than a measured injury rate. Dragging a pen produces the same lateral load, so technique can erase the difference.

Can you sterilise a derma roller at home?

No. Sterility is a state a needle leaves once and does not return to, and rinsing, alcohol and hot water do not bring it back. This is why single use cartridges exist, and why they are a running cost rather than an upsell.

Which is less painful, a pen or a roller?

Sensation varies by person, by area and by depth, and it rises as you go deeper on either format. Nobody can promise you a number, and a brand quoting a pain score of zero is describing marketing rather than nerves. Start at the shallowest setting on the most forgiving needle you own.

Does a microneedling pen help serums absorb better?

It opens a route that an intact barrier does not offer. That is a statement about the pathway and not about how much of a given formula crosses, and re:tones has measured no absorption figure for its own products. Ask any brand quoting a multiple which study produced it.

What needle length should a beginner use?

The shortest one available, on the widest needle you have, and stay there for several sessions. On the MTS that is 0.5 mm on the R-Nano cartridge. Work up between sessions rather than inside one, and treat each area of the face as its own calibration.
