Forehead Wrinkles: Why They Form, and What Can Actually Change Them

Forehead lines form where the frontalis folds the skin. How moving lines differ from lines at rest, what topical care can and cannot change, and when a clinic is the honest answer.


Forehead wrinkles are fold lines. The frontalis, the only muscle that lifts your eyebrows, pulls the forehead skin upward, and the skin creases across that pull. Early lines show only when you move, later ones stay when your face is still, and that difference decides what can change them. Below: the anatomy, the three kinds of forehead line, what creams and sunscreen can do, and when a clinic procedure is the more honest answer.

What causes forehead wrinkles?

The frontalis is a thin sheet of muscle across most of your forehead. A StatPearls anatomy review describes it as the only muscle that raises the eyebrows. Three muscles pull against it: the corrugator, the procerus and the orbicularis oculi around the eye. All four attach to skin, and the review lists forehead lines and frown lines among the creases their pull can leave. Lines form perpendicular to the direction of contraction, so a muscle that pulls up and down leaves lines that run side to side.

The skin cannot slide free of that muscle. In a study of five cadavers, Japanese surgeons found the forehead skin fixed by fibrous strands to a tissue layer that sits tight against the frontalis. When the muscle shortens, the skin goes with it, and it folds.

The fold follows a pattern. A 2020 study measured forehead skin movement in 27 volunteers during a full brow raise. In every one of them, the lower forehead moved up and the upper forehead moved down. The two movements met along one horizontal line, usually the second line counting down from the top.

Repeat that thousands of times and the pattern sets. A StatPearls review of upper face treatment traces wrinkles to repeated contraction of the muscle under the skin. Over time, that thins the dermis above it and pleats the skin. Sun and age weaken the dermis those lines are written into. The collagen side of that story is in what causes wrinkles. Horizontal lines, the 11s, and sleep lines

Not every line on your forehead has the same cause. Sort them first, and you will buy less of the wrong thing.

The 11s have their own muscles. The corrugators sit at the inner end of each brow, and a StatPearls review credits them with producing vertical frown lines. They work with the procerus, which a separate review places in the same glabellar complex. The glabella is the area between your brows.

Sleep lines are a different problem. A 2016 review separates expression wrinkles from sleep wrinkles, which differ in cause, location and pattern. Compression and shear act on the face when you sleep on your side or front. If a crease does not follow any expression you make, your pillow is a suspect.

Timing differs by sex. A 2014 study measured forehead, frown and eye-corner lines in 200 men and women aged 20 to 70. Men showed wrinkles earlier and more severely, and forehead lines were their first visible wrinkles. In women, lines around the eyes came first. Frown lines did not appear before 40 in either sex.

Is your forehead line dynamic or static?

This question decides everything else. A dynamic line appears when you raise your brows and vanishes when you stop. A static line stays when your face is completely still.

Test it in a mirror. Relax your forehead fully, then look under even light. Whatever disappears is movement. Whatever remains is etched into the skin, or it is not a wrinkle at all.

Dryness is the common look-alike. Hydration smooths the surface by flattening the valleys between skin ridges. The same review says a moisturizer's effect peaks 30 minutes to an hour after use and usually lasts about 4 hours. Why some foreheads work harder than others

Your frontalis has a practical job as well as an expressive one. The StatPearls review notes that its work goes beyond keeping the brows out of your visual field. That job gets bigger when the upper lids or brows sit low. Some people then lift their brows to see.

One small study tested the idea. Its authors start from an everyday observation: people with single eyelids often raise their brows to open their eyes wide. They followed 35 people through double-eyelid surgery in Beijing. Self-rated forehead wrinkles improved after surgery, and measurements suggested the frontalis was contracting less. The study compared people with themselves before and after, with no separate control group, and follow-up lasted three months. Treat it as a signal, not proof.

Heavy lids become more common with age. MedlinePlus notes that most drooping eyelids are due to aging, with no disease involved. It lists the exceptions too. One lid that suddenly droops or closes, or drooping with double vision or pain, needs a doctor, not a cream.

This matters for treatment. If your forehead is holding your brows up, relaxing it can let them fall. A 2022 review lists drooping of the eyelid or eyebrow among the main complications of botulinum toxin in the upper face. Its advice is planning around each person's own muscle pattern.

What topical care can do for forehead wrinkles

Topical care works on the skin, not the muscle. Within that limit, three steps have real evidence behind them.

Peptide creams are often sold as a topical stand-in for wrinkle-relaxing treatment. The best known is argireline, listed on labels as acetyl hexapeptide-8. A 2025 review notes studies reporting shallower wrinkles. It also calls the peptide's ability to reach the junction between nerve and muscle uncertain when applied to skin. Forehead patches are the other popular fix. Silicone and tape versions are sold to hold the skin still overnight. We searched PubMed and found no controlled trial of them for forehead lines.

No cream has been shown to relax the frontalis, lift brows your forehead is holding up, or fill a groove etched at rest. The wider list of what you can and cannot control is in skin aging explained.

When a clinic procedure is the honest answer

If your lines are mostly movement, or deeply etched, the tools that change them sit in a dermatologist's office, and whether any of them suits you is a conversation to have there. For wrinkles, the American Academy of Dermatology lists moisturizer and prescription retinoids alongside fillers, botulinum toxin, chemical peels, microdermabrasion and laser resurfacing. Each works on a different part of the problem.

Two cautions apply to all of it. The AAD warns against botulinum toxin at a non-medical spa, a party, a salon or someone's home. It also says product sold without proof of a medical license differs greatly from what a dermatologist uses. And if heavy lids are part of your picture, say so at the consultation. Relaxing a forehead that holds your brows up is how brows drop.

At what age do forehead wrinkles start?

It varies with sex, sun and how much you move your brows. In a study of 200 adults aged 20 to 70, men developed wrinkles earlier than women, and forehead lines were the first to show in men. Women showed lines around the eyes first. Frown lines between the brows did not appear before 40 in either sex.

Can forehead wrinkles go away without a procedure?

Not reliably. Moisturizer can soften fine dryness lines for a few hours, then they return. The retinoid trials cited here ran 16 to 48 weeks and graded overall sun damage, not forehead lines alone. Lines from movement come back every time the muscle moves, and no cream has been shown to stop the muscle.

Why do I have forehead lines in my twenties?

Usually they are movement lines. Expressive brows fold the same skin many times a day, and low-set lids or brows can add to it. Check them with the mirror test above. Daily sunscreen now protects the dermis those lines may later set into.

Do forehead wrinkle patches work?

We could not find evidence either way. We searched PubMed for controlled trials of silicone or tape patches for forehead lines and found none. A patch cannot change the muscle, and any effect on the surface would need a trial to show it.

What are the risks of wrinkle-relaxing treatment on the forehead?

A 2022 review lists drooping of the eyelid or eyebrow and uneven brows among the main complications in the upper face. It describes adverse effects as generally mild and temporary. Planning around your own muscle pattern is how clinicians try to avoid them. If you are considering it, talk to a board-certified dermatologist, not a spa.

Is sunscreen really worth it for forehead lines?

Yes, as prevention. In a 4.5-year randomized trial, adults who used sunscreen daily showed no detectable increase in skin aging. They aged 24 percent less than those who used it when they chose. That trial graded surface texture rather than forehead lines. It is still the strongest randomized evidence we found for protecting the skin those lines form in.
