How to Choose a Microneedling Pen: The Six Specs

Needle diameter, depth control, RPM, cartridge sterility, serum delivery and battery. The six specs that tell you if a microneedling pen is worth buying.


Six specs decide whether a microneedling pen is worth buying. Needle diameter and geometry. Depth control, continuous or clicked. Puncture rate. Cartridge sterility and single use. How the serum reaches the channel. Battery and motor. Read those six on any pen you are considering. Most of the rest of the listing is decoration.

What are the six specs on a microneedling pen?

They are the six numbers that change what the device does to your skin. Everything else sits downstream of them. A pen that publishes all six has measured them. A pen that publishes none has decided you will not ask.

How thin should the needle be?

Thin enough that entry displaces little tissue, and the figure should be printed in micrometres. Diameter is a real spec. It is also the spec most often confused with comfort.

Here is the honest version. Gill and colleagues tested ten volunteers in The Clinical Journal of Pain. Needle length had the strongest effect on pain. A three-fold increase in length raised the pain score seven-fold. Tip angle, thickness and width did not significantly influence pain.

So read diameter as a build-quality signal, not a comfort promise. Ask for a figure in micrometres and a needle count. If a seller answers in adjectives, that is your answer.

Continuous depth collar or clicked stops?

Continuous gives you every point in the range. Clicked gives you repeatability at fixed steps. Neither is automatically better. What matters is whether the lowest step is low enough, and whether the setting holds under pressure.

Depth earns this much attention for a reason. The layer you are crossing is thin. Gupta and colleagues put the stratum corneum at roughly 10 to 20 micrometres. Their work ran in the Journal of Controlled Release. Direct measurement agrees. Sandby-Møller and colleagues measured 71 volunteers. Mean stratum corneum thickness ran 11.0 micrometres at the shoulder and 18.3 at the forearm. The cellular epidermis underneath measured 56.6 to 81.5 micrometres.

Now put that next to a dial. The shallowest common setting, 0.5 mm, is 500 micrometres. Every setting on a microneedling pen goes well past the barrier. The dial is not choosing whether to cross it. The dial chooses how far beyond it you go. Fine control is worth paying for.

What does RPM actually change?

Puncture rate sets how many punctures land per pass. That sets how many passes you need. It is arithmetic, not a performance claim. Needles per cartridge multiplied by punctures per minute gives coverage per minute.

You do not have to argue for this spec. The federal classification for aesthetic microneedling devices is 21 CFR 878.4430. It requires that technical specifications "must be identified, including needle length, geometry, maximum penetration depth, and puncture rate." It also calls for data on penetration and puncture-rate accuracy.

A faster motor is not a better result. A faster motor means fewer passes for the same puncture density, and less dragging across skin. Distrust any number that arrives without units. The re:tones pen runs at 6,000 RPM.

Why the cartridge has to be sterile and single use

The cartridge is the part that touches you, and it dulls. Reusing it stacks two problems. Contamination is the first. A blunter tip that needs more force is the second.

Steel loses its edge faster than people expect. An electron-microscopy study in the Journal of Cosmetic Dermatology measured needle tip deformation. Mean deformation hit 14.8% after three injection passes. After ten passes it reached 29.2%. Those were injection needles, not microneedling cartridges. Treat it as a principle, not a spec. The principle holds.

The same federal classification asks for performance data on two points here. One is "the sterility of the patient-contacting components." The other is safety features against cross-contamination, "including fluid ingress protection." Remember that last phrase. Fluid travels back up a cartridge that is not sealed against it.

Check four things before you buy.

  • Are cartridges sold sterile and sealed
  • Are they labelled single use
  • How many come in the box
  • What one costs to replace

How does the serum actually reach the channel?

Three arrangements exist. You apply the serum first and needle through it. You apply it after. Or the device releases it at the tip as the needle meets skin. The third removes the timing question.

Timing is a real variable. Microneedling creates micro-conduits through the stratum corneum. That is how it enhances absorption of topical products, per the review by Iriarte and colleagues. It ran in Clinical, Cosmetic and Investigational Dermatology. Those conduits do not stay open. In the Gupta study above, unoccluded sites recovered their barrier properties within two hours.

One more point on format. You are applying to a barrier you just opened. That argues for a sealed container over a pump that meets air and fingertips daily. Our ampoules ship in 50 mL crimp-sealed medical glass for that reason.

Battery, motor and the parts nobody lists

Cordless is easier to steer. Corded holds speed. Either is fine if the motor keeps its rate when you press.

Ask five questions. How long does a full charge run. Does it operate while charging. Is the battery replaceable, or is the pen disposable at end of life. What does it weigh at the tip. Does speed sag on the highest setting. A pen that slows under pressure changes puncture density mid-pass. You will not feel it happen.

What the federal classification tells you to ask

The regulation itself is a good buying checklist. 21 CFR 878.4430 places microneedling devices for aesthetic use in class II with special controls. Those controls cover:

  • Needle length, geometry, maximum penetration depth and puncture rate
  • Accuracy of penetration depth and puncture rate
  • Sterility of patient-contacting components, and shelf life
  • Cross-contamination safety features, including fluid ingress protection
  • Biocompatibility, electrical safety and software validation
  • Labeling that states technical parameters and probable risks

Two caveats keep this honest. The classification covers devices that puncture skin for aesthetic use. It states that it "does not include devices intended for transdermal delivery of topical products such as cosmetics, drugs, or biologics." Category turns on intended use, not on how a device looks. FDA published its final guidance on which microneedling products are devices in November 2020.

Status varies product by product. Look any specific device up yourself, rather than taking a seller's word. Ours included.

Start shallow, and know when not to needle

Start at the shallowest depth, on the most forgiving cartridge, on one small area. Wait a week before you change anything. Depth is the setting with consequences.

The American Academy of Dermatology says you should not have microneedling if you:

  • Heal slowly or poorly
  • Have, or have had, a keloid
  • Have a weakened immune system
  • Are being treated for skin cancer in the area
  • Are receiving radiation treatment

The AAD also says to wait for a tan, recent sun, a skin infection, or deep pimples. It sets at least one month since a last isotretinoin pill.

The AAD is equally direct about home use. At-home devices can be overused. They can pierce skin and lead to infection or scarring. They can spread warts, herpes and other viruses across the face. Pressing too hard can damage skin. Read that as a technique warning.

Practical rules. Once a week, not more, as covered in how often to microneedle. One person, one cartridge, never shared. Clean hands, clean skin. Stop if skin is irritated, broken or inflamed. Skip retinoids around a session, per microneedling and retinol. Read who should not microneedle and microneedling aftercare before your first pass. If a doctor treats your skin, ask the doctor first.

Frequently asked questions

Is an expensive microneedling pen better than a cheap one?

Price alone tells you nothing. Published specs do. Compare needle diameter, depth range, control type, stated puncture rate, cartridge cost and battery access. A cheap pen with expensive single-use cartridges can cost more over a year. Run the refill maths before you compare sticker prices.

What needle depth should a beginner use at home?

Start at the shallowest setting your device offers. Use the most forgiving cartridge, on one small test area. Wait a full week before you change anything. Facial skin thickness varies by region, so one setting for the whole face is always a compromise. Our depth guide covers each region.

How often should you use a microneedling pen at home?

Once a week is the common home cadence. More is not better. The AAD warns that overusing at-home microneedling can irritate skin, and can make a concern more noticeable. Skin needs the interval. If redness or tenderness has not settled, wait longer rather than pushing through.

Can you reuse a microneedling cartridge?

No. Cartridges are sold sterile and single use for two reasons. Needle tips deform with repeated passes, so entry force rises and precision falls. A used cartridge also carries contamination risk. Federal special controls require sterility data for patient-contacting components, plus protection against cross-contamination.

Does a higher RPM mean a better microneedling pen?

Higher RPM means more punctures per minute. That means fewer passes for the same coverage, and less dragging. It does not mean a better outcome. What matters more is whether the motor holds its stated rate under pressure. Treat any pen with no published puncture rate as unfinished homework.

Can you use your own serum with a microneedling pen?

Physically, usually yes. Sensibly, be careful. You are applying to a barrier you just opened, so sterility and formulation matter more than usual. Avoid acids, retinoids, fragrance and essential oils around a session. Pens that release a dedicated sealed ampoule at the tip remove the guesswork.

What is the difference between a nano tip and a microneedle?

Nano tips are shallower and aim at the outermost layer. Microneedles go further in. The distinction changes what you should expect, how often you can use each, and what aftercare applies. We compare them in nano needling versus microneedling.

Do at-home pens do what a dermatologist's device does?

No, and no honest seller should say otherwise. The AAD says at-home microneedling leads to less noticeable results than treatment by a board-certified dermatologist. A home device is a different tool at a different intensity. Judge it on its specs, not on clinic comparisons.
